DescriptionThe client hospitals are situated in Riyadh, Jeddah, Dammam and Al Hasa - the mission of these four hospitals is to provide the highest quality of primary, secondary and tertiary health care services to residents of our community, the region and the surrounding Middle Eastern countries. The Hospital in Al Hasa is a 300-bed capacity and provides primary and secondary patient care.
Including: Outpatient Clinics, Pharmacy, four Operating Rooms and Day Surgery Operation Room. It also includes eight beds for Haemodialysis, Physical Therapy Department, Radiology Department, Endoscopy, ER, ICU, Obstetrics & Gynaecology Department with two rooms for caesareans and six L & D rooms. Wards include: Male and Female patient wards, Gynaecology, Obstetrics and female surgery. male surgical ophthalmology, ENT and a VIP ward.

Job Title : Clinical Pharmacist

Responsible to Immediate Supervisor

Basic Function
Within the mission, vision and values of the hospital, the Clinical Pharmacist with minimal supervision participates in clinical activities in assigned area by the Immediate Supervisor to provide Pharmaceutical Care.

Principle Duties:
1. Performs and undertakes the following duties with regards to the daily clinical activities with minimal supervision:
• Participates in medical rounds to obtain pertinent information required to determine the necessity of drug therapy, and provides input into drug selection and monitoring;
• Determines accurate Medication Related Problem list and records as necessary;
• Obtains pertinent information required to determine the necessity of drug therapy;
• Consults and liaises with Physicians with regards to drugs without indications;
• Assures the Drug of Choice for a particular patient condition is ordered;
• Selects drug products that are effective, cost-beneficial, and promotes patient compliance;
• Participates in the writing of or evaluation drug of therapy protocols;
• Suggests the appropriate therapeutic alternatives for Non-Formulatory drugs;
• Recommends dosage alteration or drug discontinuation when indicated;
• Identifies potentially significant drug-drug food/laboratory/disease interactions and communicates the clinically relevant interactions to health care practitioners with recommendations of therapeutic alternatives;
• Obtains and uses Clinical laboratory data to evaluate the appropriateness of drug product selection and/or dosing regimen and adjusts the drug therapy accordingly;
• Provides Pharmacokinetic consultation for agents requiring such monitoring;
• Provides and evaluates drug therapy orders for appropriateness of dosage, route, interval, schedule, and duration throughout the patient’s hospital course;
• Evaluates independently the patient response to drug therapy;
• Assures compliance with the ‘adverse drug reaction reporting program’ and ‘medication error- reporting programs’;
• Records recommendations, interventions or other appropriate activity in the medical record and Therapeutic Intervention Documentation program;
• Provides educational presentations to the Pharmaceutical Care Services department
2. Participates in the Pharmaceutical Care Services department graduate, undergraduate students and residents training programs.
3. Participates with minimal supervision for the Clinical Pharmacy Services that are provided within the Ambulatory Care Clinics, such as, Anticoagulation, Liver transplant, and Diabetes.
4. Performs other job related duties as required.
